Eligibility – Who Can Use Form 10D?
You may use Form 10D if you meet one of the following situations:

You’ve completed 10 years of eligible service under EPS and have reached age 58 (superannuation pension).

You are aged between 50 and 58 and have left service (you may claim a reduced pension at a discounted rate).

You are applying due to total and permanent disablement (disablement pension) before retirement.

Your claim is as a widow/widower, orphan, nominee, or dependent parent of a deceased member who was covered under EPS.
If you have not completed 10 years of service and you reach age 58, you cannot claim the full pension via Form 10D; instead you may apply for withdrawal of your pension portion via Form 10C.

9. Option for Return of Capital
Indicate if you want the “Return of Capital” option (nominee will get back capital after death) and fill nominee details (name, relationship, address, DOB).
10. Particulars of Family
List your spouse / children / nominee or dependent parents as applicable, include name, date of birth, relationship, and indicate minors or disabled children.
11. Details of Bank Account for Pension Credit
